WELCOME to the Homer Pittard Campus School HEALTH OFFICE
Welcome to Homer Pittard Campus School and our School Health Office! I am nurse Lisa Hollifield, BSN, RN. I am originally from North Carolina, but have been in the Murfreesboro area since 2006. I have been in the nursing field since 2004. The first two years of my nursing career was spent in an adult GI/Pulmonary Unit at a hospital in Spartanburg, South Carolina. I then worked at Vanderbilt Children's Hospital in Nashville, TN, in the pediatric surgery/trauma and adolescent medicine unit. I started with Rutherford County Schools in 2018 as a school nurse, and I could not be happier working with the students. This site should provide you with information and links needed pertaining to medical forms (that can be printed), frequently asked questions, and RCS policies.

*Please remember that all health forms need to be completed annually and are not able to be carried over from year to year*
*** IMMUNIZATIONS*** Parents should contact their child's health care provider or the Health Department to obtain a copy of the child's shot record. This will need to be documented on a valid "Tennessee Department of Health CERTIFICATE OF IMMUNIZATION" form prior to registration. The child must be caught up on vaccines before he/she can start school, per Tennessee law.
***HEALTH PHYSICAL*** Proof of a physical is required within the 12 months prior to starting school. This applies ONLY to students entering school for the 1st time, such as those entering Pre-K or Kindergarten and those entering RCS from a homeschool setting. The document must be provided to the school nurse within 30 days of the child's first day of school. After this deadline, your child may be excluded from school per Rutherford County policy 6.402.
